The type of personal information we collect
We currently collect and process the following information:
Personal identifiers and contacts (for example, name and contact details) at initial contact (e.g., using the Contact form).
Later, with your consent, we will also process more sensitive information about a client’s presentation including their health, education, family history, developmental history, speech and language development, social-emotional development, etc.
How we get the personal information and why we have it
The personal information we process is provided to us directly by you for one of the following reasons:
To allow us to contact you directly after completing the Contact Us form
It is gathered during the initial consultation phone call and at assessment to ensure we have a thorough understanding of the client’s presentation.
Information about the client’s progress is gathered throughout the therapy process.
We use the information that you have given us in order to inform our speech and language therapy intervention for the individual client.
We will not share any of your information unless we have permission to do so (e.g., with other professionals working with the client).
Your information is processed under the lawful bases set out in Article 6 of the General Data Protection Regulation (GDPR). We process your data only with your consent so you can take advantage of our services. You are able to remove your consent at any time by contacting us at hello@thewillowtreeslt.co.uk.
How we store your personal information
Your information is securely stored at all times. We keep all personal information for the duration you are a client with us at The Willow Tree SLT and for 1 year afterwards. We will then dispose of your information securely.
Your data protection rights
Under data protection law, you have rights including:
Your right of access - You have the right to ask us for copies of your personal information.
Your right to rectification - You have the right to ask us to rectify personal information you think is inaccurate. You also have the right to ask us to complete information you think is incomplete.
Your right to erasure - You have the right to ask us to erase your personal information in certain circumstances.
Your right to restriction of processing - You have the right to ask us to restrict the processing of your personal information in certain circumstances.
Your right to object to processing - You have the the right to object to the processing of your personal information in certain circumstances.
Your right to data portability - You have the right to ask that we transfer the personal information you gave us to another organisation, or to you, in certain circumstances.
You are not required to pay any charge for exercising your rights. If you make a request, we have one month to respond to you.
